Our online course evaluation form is attached to the WEBct program in a
quiz format that was created by Clarence Maise, Jr. our Instructional
Design Specialist.

Attached is a copy of our online SEI forms. I have also attached our
basic classroom SEI form that is created with Magenta Form Designer.

Although our college has an instructor evaluation form at this time,

online/distance education course offerings are growing and the
questions
on our existing evaluation do not reflect the intricacies of online
instruction.  Do any of you have an online/distance education
