Hi, Bill!
I administer the process at our college. We use scannable forms that
are processed by Exact-Data in Columbus. They provide individual
summary reports by instructor, discipline, division and college each
quarter. The student comments from the original forms are copied when
they are sent back to us. The instructor gets a copy of the comments,
the course report and a copy of each of the other summary reports
(discipline, division, college). The original forms and a copy of each
report, including the individual instructor summary report, go to the
division chair (the department head) for review and archive.
We use Survey Monkey for our online courses. The results of those are
not combined with the brick and mortar summaries. The division chair
gets a copy of each instructor's summary report including comments.

Subject: Evaluations of the faculty/course by the students
All,
Our students fill out an instructor and course evaluation at the end of
each quarter. Currently the instructor is able to see the results of
both the course and instructor evaluations while the department chairs
view only the course evaluations. The chairs would like to also view the
evaluations related to the instructors within the department.
So, I would like to get some feedback as to how your university handles
this "delicate" situation. Are the department chairs able to view all of
the detailed information related to their departmental faculty,
including comments, returned by the students; or are they given summary
reports? Who administers this process at your university?
